Creating a plan of shared understanding, goals, interventions and supports for a student, by the student’s circle of support, can ensure that the student receives the best possible integration of supports to, through and from the services and supports they receive. A Student Support Plan (Student Intervention Plan etc.) is one tool that may be used to enhance and ensure effective interdisciplinary collaboration.
